Participants

Take Action! is a fun and interactive 10-week worksite employee wellness program that helps participants eat more fruits and vegetables and be more physically active! Do you want to lose weight, lower your blood pressure, or simply be a good role model for your kids? We all have something to strive for to improve our health, and Take Action! can help you get there.

By participating in Take Action!, you will set your own personal fruit and vegetable or physical activity goal that you will work towards achieving every day. Not ready to do it on your own? That's ok! You will be part of a team, led by a Team Captain, who will provide you with all the support you need to meet your goal. Plus, there will be plenty of healthy activities planned during the 10 weeks and great incentives to keep you motivated.

Forms for Participants

Throughout the program, you will be asked to fill out three forms - Registration, Mid-Point Goal Re-Evaluation, and a Participant Evaluation. Remember to turn these in to your Team Captain!

Weekly Tracking Sheets

Each week you will keep track of your fruit and vegetable consumption or your physical activity minutes as you work towards a healthier lifestyle. You can use these tracking tools to help make it easier.

Educational Materials & Activities

Take Action! isn't just about eating more fruits and vegetables or being more physically active, it's also about learning why doing so is important. Learn more about the importance of fruit and vegetable consumption, regular physical activity, and common health conditions, and what you can do to improve your health!
